SOCIAL WORK CASE MANAGMENT

This course describes the competencies required to conduct social work case management. It involves assessing social work cases, developing case management plan, implementing case management plan, performing case management evaluation and preparing case management documentation    .

CONDUCT HOME BASED CARE

This course describes the competencies required to perform home based care and support. It involves identifying community health structures and partners, conducting home based care needs assessment, mobilizing required resources for home-based care, undertaking home visits, carrying out support advocacy, monitoring client’s performance and documenting home based care and support activities.
